I am employed part-time with the Ottawa-Carleton District School Board (OCDSB). The OCDSB offers psychological services free of charge to eligible students in all of its schools.
Under current Conflict of Interest and Confidentiality Guidelines, I am unable to see children/adolescents in my private practice who attend my assigned schools. If your child/adolescent attends one of the schools I serve, then I will require you to sign a waiver form acknowleding that you understand services are available to your child free of charge through his/her school, and that you have elected to retain services privately (see attached pdf form). Should your child/adolescent later enroll in a school assigned to me, I will have to recuse myself from providing any consultation services and will not be able to provide him/her with any direct services; under these circumstances, another member of the psychology department will provide follow-up services.
